To help make your visit smoother, Downtown Whitby Dentistry provides patient forms that can be completed before your appointment. These forms help the dental team review important details about your health history, dental concerns, and contact information before you arrive.

If you have questions about which forms to complete or what to bring to your appointment, please contact Downtown Whitby Dentistry before your visit.

If you have made an appointment with our office, you must complete the patient intake form the day of your scheduled appointment.

If you are moving from another dental office, kindly complete the x-ray release form to assist us in moving your dental records over.
